This program encourages our members to participate in making quilts to be given to veterans who have returned and been affected by war. Qovf acknowledges and appreciates the support of families. If your award was made at the request of a state coordinator or other request coordinator, do not fill out this form. To register a group, fill in the group name and group leader address. We also remember and honor those who have given their lives for our freedoms, but do not award qovs posthumously. It is awarded to a service member or veteran who has been touched by war. Please use this form for reporting the following types of qov awards: A tied quilt is not acceptable for a quilt of valor. Qovf awards quilts only to service members and living veterans of the u.s. Individual, block builder or lifetime member. That is a national initiative ( www.qovf.org.).
